Controlled Substance (Prescription) Refills/Renewals Policy
- Use of only one pharmacy for renewal of controlled substances.
- Controlled substances will not be mailed to pharmacies.
- Controlled prescriptions, only to be filled by this office, may only be picked up by the person they are prescribed for after verification of identity by driver’s license or Florida ID card.
- No emergency Rx will be provided for lost or stolen medications. We advise that you use a weekly pill dispenser box to improve dosing compliance.
- Renewal of controlled substances can only occur during normal business hours during a clinic visit.
- There can be no use of any other controlled substance or pain medications prescribed by another physician without informing this office.
- You must agree to a urine drug screening if requested. Substance use of any kind may result in the termination of medication management services by this office.
- Regular office visit follow-up for periodic reassessment is expected as part of the treatment plan.
- Sharing of medication or diversion of medication for other than indicated purposes can result in a felony prosecution.
- Patient understands and agrees to the FMS Controlled Substance Policy.
- A Pain management contract will be required to be signed at the onset of treatment and be updated as required by state and federal law.
